  • Date claims were extracted, period beginning and period ending.
  • Office (or location).
  • Client's name, date of birth and Medical Assistance Number.
  • Contact date.
  • Diagnosis code, procedure code and modifiers.
  • Staff name.
  • Number of units and charges per client contact.
  • Total charges per client.
  • Total units per claims transaction shown on last page.
